Disease State Management

Disease State Management defines a way of managing people at risk of, or suffering from chronic diseases such as Diabetes, Cardiovascular Disease and Asthma.

With the Australian population living longer, these diseases are becoming much more prevalent so identification and correct management of them is of particular importance. This will help to curb the increase burden of health costs in our communities and more importantly improve quality of life for a patient.

The objective of DSM is to be able to identify people at risk of these conditions and work together with them to help manage or prevent the onset of a chronic disease, rather than rely on the continual treatment of acute symptoms that result from the disease.
